Office Hours

We are open Monday through Friday from 8:30am to 5:00pm. Our office has an operating room on-site which is fully accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). Our staff speaks both English and Spanish.

New patients are always welcome. Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. Everyone at our office is at your service and both English and Spanish are spoken.

Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy on your part will make it possible to give your appointment to another patient.

The following forms can be downloaded to your computer and printed at home. In order for the doctor to provide a complete and comprehensive medical opinion it is important that he reviews all your medical information. Please be prepared to provide this information to our office along with your current medical insurance information. Below are links to our patient information forms. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
